If you or your loved one is considering a transition into a senior care community, Next Chapter Senior Transitions can help. With so many types of senior living facilities, I know it can be overwhelming trying to find the right match for you or your loved one.

That’s why I started Next Chapter Senior Transitions. My years of experience as a physician assistant treating elderly patients in senior communities throughout the Silicon Valley has made me acutely aware of the challenges and complexities of senior care.

The process of finding the right community is difficult and time-consuming, as you consider all the factors that go into that decision. Having assessed patients and provided medical care in senior communities throughout the Valley puts me in a unique position to streamline this process.

I will offer guidance at every step of the process, starting with a free consultation designed to understand you or your loved one’s situation. In fact, there is NO COST to you at any point in the process — from the initial assessment to the facility tours, recommendations, placement and post-placement follow-up.

Melette is a Stanford-trained physician assistant who has extraordinary empathy for the elderly. After spending years as a PA assessing, treating and advocating for patients in the emergency room as well as in senior communities throughout Silicon Valley, Melette is uniquely qualified to identify senior care facilities in the Valley tailored to her clients’ physical, social, and financial needs. That experience — along with caring for her elderly parents and most of her elderly neighbors — motivated her to found Next Chapter Senior Transitions in a desire to help people find the community and care they need as they transition into their next chapter.

 

How it works.

To find the right senior care community for you or your loved one, I assess your needs, research your options, tour potential communities with you, and then guide you through the often-challenging process of selecting and transitioning into a community that’s best suited for you.

 

1) Assess Your Needs & Research Options

I personally meet with you or your senior to assess your needs and priorities. During the interview, we’ll discuss every factor that plays into choosing the right senior community — including physical care needs (now and future), cognitive status, social and lifestyle preferences, location, and budget considerations. Based on your assessment, I will provide you with personalized options along with an action plan to visit the senior living communities that best fit your needs.

2) Tour Communities & Select Best Choice

Once we have a list of options, we start the process of touring the selected communities together. As your advisor, I will walk through the facilities with you, talk to the reps on your behalf, and ask specific questions designed to determine the community’s suitability for your needs. After touring all the potential communities, we will sit down together and determine which option works best for you or your loved one.

3) Move-In & Follow-Up

I will follow up with you or your senior soon after placement to make sure everyone is satisfied with the new living situation — and to ensure that your expectations of the care, comfort, and safety needs of your loved one are being met.

 

Types of care.

Next Chapter Senior Transitions will invest the time and energy required to help you find a senior community that meets your needs and desires, whether that be independent living, assisted living, or memory care.

 

Independent Living

Independent living is a lifestyle-based community for seniors who are in good health but would like to have a community of similarly aged and likeminded peers. The communities offer all of the comforts of home plus the addition of resort-style amenities, like programs and plenty of optional well-planned activities. Such communities could be shared with assisted living or more advanced care units too, allowing for an easier transition should the need arise. Depending on the community, features like exercise rooms, swimming pools, game rooms, movie theaters, and more are located onsite.

Assisted Living

Assisted living communities offer a warm, social atmosphere for seniors that provides a range of tailored assistance with daily life, while prioritizing independence and engagement. They are designed for those beginning to struggle with the everyday tasks of home maintenance and self care, such as meal preparation or medication management—or further support with personal grooming or mobility. Assisted living communities are typically arranged with common areas, outdoor space, a host of planned activities, restaurant dining and often offer private rooms, suites, or full apartments.

Memory Care

Memory care communities provide a safe alternative for seniors living with memory impairment. These facilities are staffed by specifically trained professionals to provide care for those with Alzheimer’s disease or other dementias. Their goal is to provide a supportive environment while offering opportunities to socialize and engage in activities that best suit your aging loved one’s cognitive ability, memory, attention span, language, reasoning ability and physical function. Each individual receives a personalized care plan assessing level of support needed with mobility, self care and medication management. These communities can provide a high quality of life while providing measures to ensure their safety.
